Having an understanding of your students helps a teacher to properly plan and prepare lessons as well as create lessons that are exciting and engaging for students
Here is a photo of the classroom Voices Board. Students are to focus on one of the six traits during their writing period. They are able to place a post-it note containing their number under the category on which they are concentrating. During individual writing conferences, I am able to easily glance at the Voices Board in order to know what each student is working on within their writing.

Creating clear outcomes for student performance can be achieved through targeting the various needs of a diverse group of students.
Here are copies of Fab Four reading sheets from two different levels of book clubs. Though the reading sheets are essentially asking all students to complete the same tasks, the lower level of the two groups receives sheets that prompt and guide the readers responses while the higher level of the two groups is asked to craft their responses entirely on their own.

Lessons must be structured to meet the needs of a variety of different types of learners, allow for student choice for maximum engagement, and provide opportunities for students to participate in higher level thinking.
Here is a sample of a single week's plans written in my school's format. I use these plans in order to guide the creation and preparation of my own lessons. When developing my lessons, I ensure that they are aligned with the standards and objectives found on these plans.
